Patient Terms of Use
Effective Date: July 1, 2025
Beyond Health provides online healthcare services through a secure web-based patient portal. Access to and use of this portal is subject to the following Terms of Use.
By using the Beyond Health portal, you acknowledge and agree to these Terms. If you do not agree, or are not authorized or eligible to accept them, you may not access the services.
1. User Accounts
As a registered patient of Beyond Health, you may access your personal account through the web portal. If you self-register, your account will only become active upon verification by Beyond Health.
By registering, you agree that:
You are using your true identity and not impersonating anyone else.
The information you provide is accurate, complete, and kept up to date.
Your login credentials are personal and confidential; you are responsible for maintaining their security.
You must notify Beyond Health immediately if you suspect your password has been compromised.
You are responsible for all activity under your account, including unauthorized use unless reported.
You may add up to six dependent family members (e.g., minors or adults unable to manage their own accounts) with proper consent.
Family members capable of independent access (with their own phone/email) should register separately.
You will receive essential notifications via email, SMS, or other approved channels; non-essential notifications can be disabled in your settings.
Beyond Health may deny or revoke access at its discretion.
You must seek in-person care for emergencies — the web portal is not a substitute for urgent medical attention.
You are encouraged to activate two-factor authentication (2FA); if you choose not to, you accept the associated security risks.
2. Data
All patient data is stored securely in HIPAA- and GDPR-compliant cloud infrastructure hosted on Amazon Web Services (AWS).
All data storage and transmission are encrypted.
You are responsible for the accuracy and completeness of the data you enter or upload, including data synced from health tracking devices or third-party services.
Authorized Beyond Health care providers may access and update your personal health information for the purposes of providing care.
You may upload medical records, lab reports, and related documents to your profile; these are securely stored and protected.
Non-identifiable, anonymized data may be analyzed for improving services and health outcomes.
All use of personal information is governed by the Beyond Health Privacy Policy.
3. Remote Monitoring Services
Remote Monitoring allows you to share health metrics with Beyond Health care providers but does not replace clinical visits.
All instructions from your healthcare provider must be followed accurately.
Data you enter (e.g., vitals, symptoms) must be timely and accurate.
You are responsible for using reliable, calibrated medical devices.
Your provider may assign qualified healthcare professionals to monitor your data.
Monitoring is performed at specified intervals and may vary depending on your care plan.
Remote Monitoring is not an emergency care service. In urgent situations, you must contact emergency services or your nearest hospital.
Failure to comply with instructions may result in termination of services.
You may discontinue participation in a Remote Monitoring plan at any time with your provider’s knowledge.
Your health data remains confidential and will only be shared when legally required or in emergencies.
4. Text or Question-Based Consultations
Efforts will be made to respond to your questions in a timely manner; however, Beyond Health does not guarantee specific response times.
Submitting a question does not automatically establish a doctor-patient relationship.
All responses are for informational purposes and must be verified with your primary care provider before changing treatment.
You must not rely on these responses in emergency situations — seek immediate in-person medical attention when needed.
Information provided by you must be accurate to ensure meaningful and relevant feedback.
Each follow-up note or question may require standard response times, similar to your original inquiry.
All applicable fees for this service are subject to the posted payment terms.
5. Payments
Fees for services provided via the Beyond Health web portal must be paid according to the terms posted at the time of use.
Offline payments may be supported — contact Beyond Health for more information during your next visit.
Service pricing may change at the discretion of the healthcare provider.
Payments for Remote Monitoring plans are non-refundable, even if you choose to discontinue before the plan period ends.
No financial data (such as credit card information) is stored by Beyond Health.
6. Privacy Policy
Your use of the web portal is also governed by Beyond Health's Privacy Policy. Data collected through the portal includes:
Personal Information: Name, email, phone number, address, and optional profile photo.
Health Information: Age, gender, medical history, medications, allergies, and synced health tracker data.
Operations Data: Device info, browser type, IP address, timestamps, and general usage data.
Cookies: Used for authentication and session management. You may disable cookies in your browser, though some features may not function correctly. Only necessary cookies (e.g., AWS load balancer) are used.
Storage & Security: All data is encrypted and stored in secure, firewall-protected infrastructure. No data is stored locally on your device.
You may request deletion of your account at any time. Upon confirmation, all associated data will be permanently removed except for billing records, which are retained for compliance.
In case of any personal data breach, you will be notified in accordance with applicable laws.
7. Use of Information
Your personal and health information is only accessible by authorized Beyond Health staff and healthcare providers involved in your care.
Beyond Health will never disclose your information to third parties except when required by law or with your explicit permission.
Anonymized, aggregated data may be analyzed for improving services, but it will never include personally identifiable information.
You are responsible for maintaining your account credentials securely. Beyond Health is not liable for any unauthorized use resulting from compromised login information.
The technology vendors supporting this portal are not responsible for data access policies — that responsibility lies solely with Beyond Health.
8. Disclosure of Information
Beyond Health may disclose your information:
To comply with legal obligations or law enforcement requests.
To prevent fraud, abuse, or threats to the safety of individuals.
With your explicit consent or upon your request.
In response to a valid emergency involving your health and safety.
Users found violating these Terms of Use may have their accounts suspended or permanently deleted.
9. Transmission & Storage Security
All data transmissions are encrypted (e.g., TLS/SSL) and stored in secure AWS environments compliant with HIPAA and GDPR.
While strong measures are in place, no system is 100% immune to breaches. By using the portal, you accept the residual risks of data transmission over the internet.
10. Age Restrictions
The Beyond Health patient portal is intended for users age 18 and above.
Parents, caregivers, and legal guardians may create and manage records on behalf of minors or dependent adults with proper consent.
The individual creating and managing such dependent profiles assumes full responsibility for the data shared.
11. Changes to These Terms
Beyond Health may revise these Terms of Use periodically. Any updates will be posted here and will take effect immediately upon publication. Continued use of the portal after changes are made constitutes acceptance of the updated terms.
